What is UDS?
The Uniform Data System (UDS) is an annual reporting system that provides standardized information about the performance and operation of health centers delivering health care services to underserved communities and vulnerable populations.
How is RHG different from other providers?
We are a community-owned and operated non-profit health care center. We can discount our fees for each patient based on family size and income. We are a family practice. That means that we see patients of all ages. We have pediatricians on staff to help our youngest patients, although every site has providers who can see children. We also have health promotion programs to help people manage their weight, their diabetes and other chronic health problems. In most cases we can offer you a same-day appointment.
Is the healthcare center only for people without health insurance?
We are a community health center, serving the medical and dental needs of all types of people in our community. We will certainly see people who have no health insurance. We also take almost all types of medical insurance, including Medicaid and Medicare.
Are you a free clinic?
Rural Health Group is not a “free clinic” – however, our services are available at a deeply reduced fee to those who qualify based upon household size and income. If you qualify for the Sliding Fee Discount Program, we will not deny service due to inability to pay. If you have trouble paying for your health care due to your financial situation, please contact your provider’s office for questions.
What is Sliding Fee?
The Sliding Fee Discount Program is a federal program that permits Rural Health Group Inc. to discount normal charges for either a medical or dental visit. According to law, it requires two pieces of information in order to qualify; the amount of income in the household and the number of people who live in the household. In order to be eligible for the Sliding Fee Discount Program, you must provide accurate and acceptable proof of income as well as list all persons in the household. These forms of documentation must be presented upon applying for the Sliding Fee Discount Program. Failure to provide such documentation in a timely manner will cause a delay in processing your application and you will be responsible for 100% of all charges including any lab work performed. You must report any changes in family income or number of member in the household when these changes occur. Falsification of this information will result in forfeiture of the Sliding Fee Discount Program privileges and possible release from the practice as it is a violation of Federal Law. Sliding Fee information must be updated every twelve (12) months or with any change of household income or household size.
